3. POLICY
3.1 Patients who meet the criteria set forth in this procedure shall be considered a replant center candidate and shall be transported directly to a Replant Center, with the approval of the Citywide Dispatch Supervisor.
3.2 Unstable or Multi-Trauma patients with compromised vascular status, regardless of the level of injury, shall be treated and transported in accordance with applicable patient care protocols. Following stabilization at the appropriate hospital, transfer of these patients to a Replant Center should be considered.
3.3 REPLANT CENTER CRITERIA - patients with stable hemo-dynamics without the use of M.A.S.T. and:
3.3.1 A total, partial or isolated amputation of a body part including, but not limited to:
3.3.2 Arm, forearm, wrist, hand, finger, thumb, thigh, calf, ankle, foot, toe, scalp, ear, nose,
penis, or;
3.3.3 A crushing injury, an avulsion or a severe/deep laceration to a body part that results in
the distal portion being:
A. Cyanotic
B. Pulseless
C. Pale or
D. Cool to the touch.
4. PROCEDURE
4.1 Upon identification of a Replant Center Candidate, members shall:
4.1.1 Provide treatment in accordance with applicable NYC REMAC patient care protocols, including, but not limited to, NYC REMAC BLS Protocol 426, Soft Tissue Injuries.
4.1.2 Ascertain from the dispatcher the availability of replant center resources.
4.1.3 Transport the patient and the body part(s) together to the Replant Center.
4.1.4 Consider and/or request the use of a Medevac, if appropriate.
