REPORTED SEPARATION OF THE HEAD HARNESS

SOME USERS OF AV-3000 FACEPIECES HAVE REPORTED SEPARATION OF THE HEAD HARNESS
FROM THE FACEPIECE AT THE RUBBER ATTACHMENT TAB. SEPARATION OF THE TAB MAY
RESULT IN EXPOSING THE USER TO THE ATMOSPHERE THE RESPIRATOR WAS INTENDED TO
PROTECT THE USER AGAINST.

This (or one's very similar) have come out several times over the last few years for both the AV-2000 and AV-3000 masks. I've been told it is usually just a corporate CYA measure. However this is yet another reminder to check your mask at the start of every shift!
