E235: URGENT URGENT URGENT! We have a 4 story building totally collapsed right in front of us!  Myrtle and Ryerson!

Battalion 57: Urgent, 10-84, the building just collapsed as we pulled up, 10-60, we're unsure if there's people in there!  We're taking of rollcall of members at this time!

BC57: We're using All-Hands, we still need all units to continue in.  The building was a 4 story brick, address 493 Ryerson Ave.  All FD members are accounted for at this time, unknown if there are victims!

Exposures are:
1 - street
2 - similar attached
3 - rear yard
4 - similar attached.

Division 11: We're 10-84, we need a forthwith response from the utilities.

BC57: At this time we have 2 lines stretched precautionary.  Exposure 2 has been fully evacuated.  Exposure 4 is being evacuated.  Possibly 3 people unaccounted for.

FieldCom: Progress report for your 10-60 2nd Alarm Box 633, address 495 Myrtle Ave off Hall St, Collapse of a 4 story brick 20x40.  At this time, Car 9, DAC Sudnik reports: all void searches have been completed.  Utilities have been shut down by the Rescue Company.  At this time companies are searching from the exterior.  Incident is still Doubtful.  Also have PD Supervisor report to the command post in the front of the building.

As per Transit IND Dispatcher 924, there are no train tunnels under the collapse area.

MCC1: At your 2nd Alarm/10-60 box 633, address 495 Myrtle Ave at Hall St, Collapse of a 4 story building 20x40, at this time, Car 9, DAC Sudnik reports: All occupants of the building have been accounted for.  There were a total of 6 including the building owner.  All void searches have been suspended.  At this time we are awaiting a grappler to come remove parts of the building so we can do further void searches.  All searches have been suspended due to the accountability of all occupants.  By orders of Department of Buildings, Exposures 2, 2A, 2B, 2C, and exposure 4 are being evacuated, we will get back to you with the number evacuated, and Car 9 places this incident Probably Will Hold.

MCC1: At this time by orders of Car 9 change the address of the building to 493 Myrtle Ave.

MCC1: At this time Car 9, DAC Sudnik would like 1 engine, 1 tower ladder, and 1 Battalion for relief.

MCC1: Progress report for your 2nd Alarm 10-60 Box 633, address 493 Myrtle Ave, Collapse of a 4 story 20x40.  Car 9, DAC Sudnik reports: At this time we're going down to 1 engine, 1 truck, 1 TAC unit, and 1 Battalion.  At this time we're awaiting Department of Buildings to assess damage to exposure 2.  Also place this incident Under Control.
