Fireman Macari died of smoke inhalation while operating at a single alarm fire.

Fireman Caesar J. Macari of Ladder 128 was operating on the roof at the base of a two-hundred foot electrical sign when he was enveloped in smoke. Once the smoke passed he was seen staggering and then collapsed. An inhalator was brought up to the roof and Macari was worked over for a half hour before being brought down the ladder, taken to a store and worked on more. He was pronounced dead in the store. He was the father of three children and appointed to the Department in July 1931. The three-alarm blaze had destroyed twenty stores along Greenpoint Avenue, between 45th and 46th Streets. Fireman Macari is buried in Third Calvary Cemetery in a grave that lies less than 2,000 feet from the site of the fire that took his life. (from The Last Alarm, by Boucher, Urbanowicz & Melahn)

RIP. Never forget.
